These forms are in Adobe PDF format and each is designed to print on 8 1/2 by 11 paper. These electronic forms contain form fields so they can be completed by entering information directly to the electronic form while online or the form can be downloaded to your PC to be completed offline. As an alternative, a blank copy of the form can be printed and completed by handwriting the information on the form. Once completed, 2 copies of the forms and supporting documents are mailed to the plan as usual.
